This form doesn’t actually sign you up for a headshot. It begins a dialog process that will allow us to create a
schedule that fits both of our needs. 

I will eventually need your address information to mail the final disks to you, and I will need your email address
and phone number to communicate with you about the shoot.  The information you supply to me will never be
sold or given to anyone else.

I usually limit my headshot photography to alternating Saturdays, but in special cases, I can shoot throughout the week.
I find that Saturdays work out very well for most people’s schedules, but if you need another day, please mention it in the
text field supplied.
